## Key Responsibilities
• Manage a dedicated patient panel of 1,800 established patients

• Collaborate with a pharmacist on medication reviews for high-risk patients (15-20 monthly)
• Utilize Epic EMR for documentation with 99.2% clean claim rate

• Conduct 4-5 diabetic patient visits daily with HbA1c tracking

## Requirements
