Experienced NP or PA Needed for Value-Based Primary Care in Bradenton, FL
A growing, physician-led healthcare organization in Bradenton, FL is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Value-Based Care, Managed Care, or Medicare Advantage to join our outpatient primary care team.
This is an excellent opportunity for an experienced provider who enjoys delivering relationship-focused care to adult and geriatric patients in a collaborative, quality-driven environment.
Providers will work closely with physicians and clinical support teams to improve patient outcomes while managing a reasonable daily patient volume.
Position Highlights
- 100% outpatient primary care
- Monday–Friday schedule
- No hospital call
- Average patient volume: 12–17 patients per day
- Established Medicare Advantage patient panel
- Team-based value-based care environment

Responsibilities
Primary Care Services
Provide comprehensive outpatient care for adult and senior patients including:
- Preventive care
- Chronic disease management
- Health assessments
- Medication management
- Follow-up care
Value-Based Care Management
Deliver evidence-based care focused on improving quality metrics, patient outcomes, and preventative health initiatives.
Documentation & Compliance
Maintain accurate EMR documentation and support quality initiatives including HEDIS measures and Medicare Advantage standards.
Independent Clinical Practice
Work independently while collaborating with physicians, care coordinators, and support staff in a fast-paced outpatient environment.
Qualifications
- Active Florida NP or PA license required
- Minimum 3 years of NP/PA experience preferred
- Prior experience in:
- Managed Care
- Medicare Advantage
- Value-Based Care
- Primary Care
- Experience with HEDIS measures strongly preferred
- Strong communication and patient care skills
- Comfortable working independently
