A physician-led, value-based care organization is seeking a Board Certified or Board Eligible Family Medicine or Internal Medicine Physician (MD/DO) to join its growing outpatient primary care practice in Wildwood, Florida.

• Salary: $240,000 – $300,000 Base + Performance Incentives
• Monday–Friday, 8:00 AM – 5:00 PM
• 100% Outpatient Primary Care

Clinical Responsibilities

• Provide comprehensive outpatient primary care for adult and senior Medicare Advantage patients
• Perform Annual Wellness Visits
• Diagnose and manage acute and chronic medical conditions
• Manage hypertension, diabetes, COPD, heart disease, and other chronic illnesses
• Develop individualized treatment plans
• Coordinate care with an interdisciplinary healthcare team
• Utilize HEDIS quality measures within a value-based care model
• Maintain accurate and timely EMR documentation

Qualifications

• MD or DO
• Board Certified or Board Eligible in Family Medicine or Internal Medicine
• Active Florida Medical License (or ability to obtain)
• Interest in Value-Based Care and Medicare Advantage
• Managed Care experience preferred but not required
