Clinical Role & Responsibilities
As the lead of our team-based medical group, you will focus on Value-Based Care for the Medicare Advantage population.
- Chronic Disease Management: Diagnose and treat seniors with multiple comorbidities and complex care needs.
- Preventative Health: Lead outpatient medicine initiatives focusing on health maintenance and disease prevention.
- Care Coordination: Communicate effectively with the care team and families to avoid fragmented care or unnecessary testing.
- Leadership: Serve as the clinical lead for a multidisciplinary support team.

Requirements:
- Must be Bilingual (Spanish/English).
- Board Certified or Board Eligible in Internal Medicine or Family Medicine.
- Experience or strong interest in Geriatric / Managed Care models.
- Active Florida Medical License.
