A physician-led healthcare organization is seeking a Board Certified or Board Eligible Internal Medicine or Family Medicine Physician to join its growing value-based care practice in Fort Pierce, Florida.

This opportunity is ideal for a physician who enjoys practicing relationship-centered medicine, developing long-term patient relationships, and building a thriving patient panel within a quality-focused care environment.

The incoming physician will play a key role in expanding access to care within the community while helping grow an established Medicare Advantage patient population.

This position requires a physician who is comfortable practicing independently within the clinic setting while collaborating with a larger regional network of providers and support teams.

Bilingual Spanish fluency is required.

Responsibilities

Comprehensive Outpatient Primary Care

Provide high-quality primary care services to adult and geriatric patients within a value-based care environment.

Patient Panel Growth & Development

Build and maintain a growing patient panel through exceptional patient care, community engagement, and continuity of care.

Chronic Disease Management

Manage common chronic conditions including:

  • Diabetes
  • Hypertension
  • COPD
  • Heart Disease
  • Hyperlipidemia
  • Other chronic medical conditions

Preventive Medicine

Develop personalized care plans focused on wellness, prevention, and improved long-term patient outcomes.

Care Coordination

Collaborate with specialists, care coordinators, and support staff to ensure seamless patient care and successful quality outcomes.

Documentation & Compliance

Maintain accurate EMR documentation while meeting organizational quality and regulatory standards.

Qualifications

Required

  • MD or DO
  • Board Certified or Board Eligible in:
    • Internal Medicine (IM)
    • Family Medicine (FM)
  • Active Florida Medical License or Ability to Obtain
  • Bilingual Spanish Required
  • Strong Communication Skills
  • Commitment to Patient-Centered Care

Preferred

  • Experience with Medicare Advantage populations
  • Value-Based Care experience
  • Population Health experience
  • Interest in growing and developing a patient panel
