A growing physician-led healthcare organization is seeking a Primary Care Physician, Nurse Practitioner, or Physician Assistant to join its expanding value-based care team serving the Sanford area and surrounding communities.

Responsibilities

Outpatient Primary Care

Develop and manage individualized treatment plans for patients with chronic medical conditions including diabetes, hypertension, COPD, heart disease, and related illnesses.

Preventive Care

Promote wellness, preventive screenings, vaccinations, and early intervention strategies to improve patient health and quality metrics.

Team-Based Care

Collaborate with physicians, advanced practice providers, care coordinators, specialists, and support staff to optimize patient outcomes.

Qualifications

Required

✅ MD or DO

OR

✅ Nurse Practitioner (NP)

OR

✅ Physician Assistant (PA)

✅ Active Florida License or Ability to Obtain

✅ Strong Communication Skills

✅ Commitment to Patient-Centered Care
