Physician Responsibilities

Outpatient Primary Care

Provide comprehensive primary care services to adult and senior patients in a modern outpatient setting focused on continuity of care and long-term patient relationships.

Chronic Disease Management

Manage conditions including:

  • Diabetes
  • Hypertension
  • COPD
  • Coronary Artery Disease
  • Hyperlipidemia
  • Congestive Heart Failure

Preventive Medicine

Perform annual wellness visits, preventive screenings, medication management, and patient education focused on improving long-term health outcomes.

Team-Based Care

Work closely with nurse practitioners, physician assistants, care coordinators, and support staff to deliver coordinated, patient-centered healthcare.

  • MD or DO Degree
